The thought of the dentist’s chair might make most people shudder but cosmetic dentistry is now almost as popular as nips and tucks. Here’s what’s new.
Every year plastic surgery becomes more popular and people are willing to spend thousands of pounds to transform their bodies. But now smile makeovers are starting to catch up.
With a new set of teeth often costing less than drastic surgery, and with fewer complications, soon we could all have perfect Hollywood smiles.
Dr Thang Nghiem, 34, from UltraSmile dentists, UK practice of the year, says a new smile can change your whole outlook on life.
He says: “There has been an increase in interest about cosmetic dentistry, probably due to all the makeover shows on TV.
“Studies have shown that people link nice smiles with success, health and intelligence. And others indicate a smile makeover is now replacing a facelift.
“A great smile takes attention away from other parts of your face and gives you a huge confidence boost.
“If people have a makeover with crowns, veneers and all sorts it can be expensive but a smile improvement doesn’t have to be that much, it can be as simple as whitening or fixing a chipped tooth.”
Small treatments can cost just £50 while extensive procedures can run into hundreds and sometimes thousands of pounds.
So if you want to change your smile, here’s everything you need to know…
The wand
“This is perfect for patients who are afraid of needles,” says Dr Nghiem, “it’s a painless injection system. “The pain from an injection isn’t from the needle going into the gum, but from the pump of the solution going in. “The wand controls the delivery of the anaesthetic so it’s very slow, drop by drop and the needle’s thin.”
Invisalign braces
These are originally from the US and are an alternative option to metal braces. Dr Nghiem, who trained in the US and UK, says: “They are clear transparent trays that you wear day and night.
“However you do take them out for eating, drinking and brushing your teeth so it’s better for hygiene than permanent braces. “It’s so popular because it’s virtually invisible when you’re wearing it.”
Inman aligners
If only a few of your front teeth need straightening then these braces are perfect. Dr Nghiem says: “They only treat the front six teeth, not the back ones. They’re also removable with a clear front piece so again, it’s relatively invisible.”
Whitening
Tooth whitening can be done in two ways but both add a gel of hydrogen peroxide.
Home whitening uses custom-made trays that you wear at night and fill with the gel. It usually takes one or two weeks to work.
The alternative is in-surgery whitening. Dr Nghiem says: “It’s put directly on the teeth and then we shine a light on it to activate it, which takes around an hour and a half.
“That creates an instant effect and for the best results it’s best to start with an in-surgery treatment followed by home whitening.
“Your teeth whitening effect will be temporary though, usually one to two years depending on what you eat.
“There’s a misconception that you’ll end up with a Hollywood smile from whitening, but that won’t happen. However, it will make them look cleaner and brighter.”

Porcelain veneers are used to change the shape, colour and often the position of teeth. They’re used for closing gaps in your smile, too and the front of the tooth is filed down first.
“Most Hollywood smiles have had porcelain veneers fitted,” Dr Nghiem says. “If for instance your teeth stick in and you need to make them look in line with other teeth, we can place thick porcelain veneers on them.
“From the front it will look like they are in line with everything else. It gives the impression that the teeth have changed position, even though they haven’t really.
“Veneers are very thin pieces of porcelain bonded directly to the front of the tooth. They’re like false fingernails for teeth. But once they’re on they are glued on permanently.
“The ideal outcome is for you not to notice them, and for them to feel natural when you eat and talk.”
Crowns
Crowns are another way of changing the colour and shape of teeth but they require a lot more work than veneers as the teeth have to be filed to stumps. “For crowns both the front and back of the tooth has to be filed,” says Dr Nghiem.
“Crowns are necessary when the teeth are very short or very heavily filed so there’s not much tooth left. Veneers are attached to the front of teeth while crowns cover the entire tooth.”
Missing teeth
There are three ways to replace missing teeth – dentures, bridges and implants.
Dr Nghiem says: “When someone loses a tooth a dentist will check everything is OK but often they aren’t told that, in the gap, both the bone and gum shrink, and will continue to shrink throughout your life. It can cause your face shape to change.
“A denture is usually the worst option as it irritates the gum and isn’t very stable. Then there’s a bridge, which goes on the two teeth either side of the gap. It looks really nice and works well but the downside is that you have to file down the teeth either side.
“People can also have implants, which are the best option for replacing teeth because you don’t touch the two teeth at the side, and they have around a 95 per cent success rate after five to 10 years.
“We make a small nick in the gum and then drill into the bone. It sounds worse than it is! It’s painless and there are minimal complications.
“The implant is a titanium screw that goes into the bone and it’s left for 4-6 months to heal and then we put on a normal crown. Once it’s in it’s permanent and stops the bone and gum from shrinking.”

“Crowns are necessary when the teeth are very short or very heavily filed so there’s not much tooth left. Veneers are attached to the front of teeth while crowns cover the entire tooth.”
Missing teeth
There are three ways to replace missing teeth – dentures, bridges and implants.
Dr Nghiem says: “When someone loses a tooth a dentist will check everything is OK but often they aren’t told that, in the gap, both the bone and gum shrink, and will continue to shrink throughout your life. It can cause your face shape to change.
“A denture is usually the worst option as it irritates the gum and isn’t very stable. Then there’s a bridge, which goes on the two teeth either side of the gap. It looks really nice and works well but the downside is that you have to file down the teeth either side.
“People can also have implants, which are the best option for replacing teeth because you don’t touch the two teeth at the side, and they have around a 95 per cent success rate after five to 10 years.
“We make a small nick in the gum and then drill into the bone. It sounds worse than it is! It’s painless and there are minimal complications.
“The implant is a titanium screw that goes into the bone and it’s left for 4-6 months to heal and then we put on a normal crown. Once it’s in it’s permanent and stops the bone and gum from shrinking.”

A perfect smile

Constructing an aesthetically pleasing smile is now easier due to the advances in aesthetic dentistry.
A BRILLIANT smile lights up anyone’s day, especially if the recipient of the smile gets to see perfect, pearly whites.
However, not many of us are born with straight, perfect pearly whites.
As a result of the combination of our genes and the way we treat our teeth, they can become stained, decayed, broken, crooked, twisted, loosened and even missing; while our gums can recede or conversely, look too prominent.
Driven by patient demand, the field of aesthetic dentistry has gained much prominence in recent years. Due to advances in dental technology and materials, coupled with a better understanding of the various clinical problems, dentists can now create more aesthetically pleasing smiles.
What constitutes an attractive smile?
When designing smiles, aesthetic dentists aim to have teeth shapes and dimensions to be proportionate with one another and in harmony with the surrounding gum.
This concept is called optimising the White (referring to teeth) and Pink (referring to gum) aesthetics. This is important as each complements the other.
Therefore, to design a smile to be more proportionate, symmetrical and harmonious, the aesthetic dentist may do one or more of the following:
1. Reposition crooked and twisted teeth
2. Reshape disproportionate teeth
3. Re-shade stained/yellow/dark teeth
4. Replace missing teeth
5. Reposition the gum line
6. Reshape the gum
There are various treatment options available to achieve the aforementioned objectives.
Orthodontics (braces)
This involves the use of metal wires and brackets attached to teeth to correct or align crowded teeth and uneven bite.
It can also be used to close gaps or for minor tooth alignment to enhance a person’s smile.
The single most important advantage of using braces is that it leaves the natural teeth intact. However, tthe treatment involves regular follow-up over a period of one to two years. Nonetheless, I must stress that this is still the best option for correcting over-crowded teeth and minor gap closure.
The good news is ceramic aesthetic brackets are now available to replace the “eye-catching” metallic brackets.
Porcelain veneers
Porcelain veneers are thin, custom-made ceramics designed to cover the front side of teeth. They’re made by a dental technician, usually in a dental lab, working from a model provided by your dentist.
You should know that this is usually an irreversible process. Typically, the teeth receiving porcelain veneers must first be lightly trimmed or prepared to make room for the veneers.
After the teeth are prepared, the dentist makes an impression for the dental laboratory which fabricates the veneers on a stone model made from the impression.
At the next appointment, the finished veneers are tried in the mouth to assess the colour, form, and fit. Then each veneer is bonded to the prepared tooth with composite resin cement.
Once bonded to the enamel surfaces of the tooth, the porcelain veneer is very strong, although patients are still advised to avoid biting hard foods or objects, to prevent chipping.
Porcelain veneers are used to close small gaps in your front teeth or correct teeth that are stained, badly shaped or crooked. Teeth with large fillings may not be suitable for porcelain veneers and may need to be treated with crowns.
Alternative treatments for teeth that might be treated with porcelain veneers include tooth whitening (for yellow or mildly discoloured teeth), direct bonding with composite resins (for slightly spaced teeth and minor aesthetic corrections), and full coverage with crowns (for extensively restored teeth and more complex aesthetic modifications).
Porcelain crowns
Crowns are dental restorations that cover the entire tooth on which they are cemented. They are indicated for rebuilding broken or decayed teeth and restore them to their original size and shape. They are also used to strengthen teeth with large fillings and very little sound tooth structure left.
Crowns can be either porcelain-fused-to-metal crowns (PFM crowns) or full porcelain crowns. The former consists of porcelain supported by a metal coping on the inner surface to combine the aesthetics of porcelain with the strength of the metal. While this has been very popular in the past, it is losing its appeal due to its poor aesthetics as a result of the metal coping which blocks light transmission through the underlying tooth. This optical setback gives the PFM crowns a very opaque and unnatural appearance.
While full porcelain crowns offer superior aesthetics due to their translucent nature and similar optical properties with natural tooth structure, they were shunned in the past for lack of fracture toughness or mechanical strength.
However, the advances in material science and dental technology has changed this.
The present generation of full porcelain crowns are very durable and have fracture toughness that can well withstand normal chewing forces. It is now possible to achieve very life-like and durable restorations with full porcelain crowns.
This option is made even more attractive by the escalating costs of gold, which has made the PFM crowns very expensive. Note that I am assuming gold alloy, as professionally recommended, is being used as the metal in PFM crowns.

Driven by patient demand, the field of aesthetic dentistry has gained much prominence in recent years. Due to advances in dental technology and materials, coupled with a better understanding of the various clinical problems, dentists can now create more aesthetically pleasing smiles.
What constitutes an attractive smile?
When designing smiles, aesthetic dentists aim to have teeth shapes and dimensions to be proportionate with one another and in harmony with the surrounding gum.
This concept is called optimising the White (referring to teeth) and Pink (referring to gum) aesthetics. This is important as each complements the other.
Therefore, to design a smile to be more proportionate, symmetrical and harmonious, the aesthetic dentist may do one or more of the following:
1. Reposition crooked and twisted teeth
2. Reshape disproportionate teeth
3. Re-shade stained/yellow/dark teeth
4. Replace missing teeth
5. Reposition the gum line
6. Reshape the gum
There are various treatment options available to achieve the aforementioned objectives.
Orthodontics (braces)
This involves the use of metal wires and brackets attached to teeth to correct or align crowded teeth and uneven bite.
It can also be used to close gaps or for minor tooth alignment to enhance a person’s smile.
The single most important advantage of using braces is that it leaves the natural teeth intact. However, tthe treatment involves regular follow-up over a period of one to two years. Nonetheless, I must stress that this is still the best option for correcting over-crowded teeth and minor gap closure.
The good news is ceramic aesthetic brackets are now available to replace the “eye-catching” metallic brackets.
Porcelain veneers
Porcelain veneers are thin, custom-made ceramics designed to cover the front side of teeth. They’re made by a dental technician, usually in a dental lab, working from a model provided by your dentist.
You should know that this is usually an irreversible process. Typically, the teeth receiving porcelain veneers must first be lightly trimmed or prepared to make room for the veneers.
After the teeth are prepared, the dentist makes an impression for the dental laboratory which fabricates the veneers on a stone model made from the impression.
At the next appointment, the finished veneers are tried in the mouth to assess the colour, form, and fit. Then each veneer is bonded to the prepared tooth with composite resin cement.
Once bonded to the enamel surfaces of the tooth, the porcelain veneer is very strong, although patients are still advised to avoid biting hard foods or objects, to prevent chipping.
Porcelain veneers are used to close small gaps in your front teeth or correct teeth that are stained, badly shaped or crooked. Teeth with large fillings may not be suitable for porcelain veneers and may need to be treated with crowns.
Alternative treatments for teeth that might be treated with porcelain veneers include tooth whitening (for yellow or mildly discoloured teeth), direct bonding with composite resins (for slightly spaced teeth and minor aesthetic corrections), and full coverage with crowns (for extensively restored teeth and more complex aesthetic modifications).
Porcelain crowns
Crowns are dental restorations that cover the entire tooth on which they are cemented. They are indicated for rebuilding broken or decayed teeth and restore them to their original size and shape. They are also used to strengthen teeth with large fillings and very little sound tooth structure left.
Crowns can be either porcelain-fused-to-metal crowns (PFM crowns) or full porcelain crowns. The former consists of porcelain supported by a metal coping on the inner surface to combine the aesthetics of porcelain with the strength of the metal. While this has been very popular in the past, it is losing its appeal due to its poor aesthetics as a result of the metal coping which blocks light transmission through the underlying tooth. This optical setback gives the PFM crowns a very opaque and unnatural appearance.
While full porcelain crowns offer superior aesthetics due to their translucent nature and similar optical properties with natural tooth structure, they were shunned in the past for lack of fracture toughness or mechanical strength.
However, the advances in material science and dental technology has changed this.
The present generation of full porcelain crowns are very durable and have fracture toughness that can well withstand normal chewing forces. It is now possible to achieve very life-like and durable restorations with full porcelain crowns.
This option is made even more attractive by the escalating costs of gold, which has made the PFM crowns very expensive. Note that I am assuming gold alloy, as professionally recommended, is being used as the metal in PFM crowns.

According to reports, most people in the UK aren’t keen on the ultra-bright look sported by many celebrities. Dr Oliver Harman from the British Academy of Cosmetic Dentistry was quoted as saying, “We want them straighter, we want them lighter but we don`t want to go beyond what you`d normally see in nature.”
He went on to indicate that the extra white celeb look is hard to accomplish by bleaching alone, but rather through the use of crowns or porcelain veneers. Referring to the trend as "that almost Barbie look," Dr. Harman stated that most Brits preferred to have nice looking, healthy teeth.
